Overview
The Health Advocate reports to the Study Director and works closely with the research team and patient care teams. In collaboration with the team, the health advocate actively engages with and coordinates the recruitment, enrollment, and follow-up for participants who require assistance managing their blood pressure and enroll in PRESSURE CHECK, a multi-year study carried out in community settings (including Yale New Haven Hospital). The study seeks to improve the quality of health care offered and to help prevent/manage hypertension outcomes following community-engaged approaches. This position will be located at 63 York Street, New Haven. This position needs someone who is fluent in both English and Spanish, as it is mandatory for the role.
Key Responsibilities:
Recruitment & Enrollment:
Professionally recruits, screens blood pressure, and interviews community members. Organizes time to engage as many potential and active participants as possible during an assigned shift, reviews questionnaires, sets priorities, and provides appropriate interventions and referrals. (Note: Study recruitment is scheduled to end August 31, 2026, but recruitment may extend through October 31, 20206, contingent on meeting enrollment goals. Participant follow-up will continue through August 31, 2027).
Communication & Collaboration:
Effectively communicates with potential and active study participants, healthcare providers, and research staff regarding plans for intervention and referrals. Consults with the research team, PCPs, pharmacy, and CHWs.
Data & File Management:
Processes, documents, and accurately enters information into the database (REDCap/Epic) to ensure project records are organized and accessible while maintaining strict participant confidentiality. Ensures adequate enrollment supplies (blood pressure cuffs, informational materials) are available at community-based organizations.
Monitoring & Follow-up:
Arranges and documents follow-up appointments and phone calls. Tracks study activities and regularly informs leadership of progress.
Community Relations:
Recommends ideas to enhance the study. Exhibits excellence in customer relations with a high level of sensitivity to the varying needs and communication styles of culturally diverse patients and partners. Participates in required meetings and trainings.
Required Skills and Abilities
1. Interpersonal & Communication Skills: Excellent ability to engage, build rapport, and demonstrate empathy/sensitivity with culturally diverse populations and partners.
2. Organizational & Data Accuracy: Strong attention to detail with proven ability to maintain accurate documentation, manage participant files, and perform precise database entry.
3. Adaptability & Independence: Ability to work independently, collaborate effectively within a team environment, and maintain a flexible schedule (including evenings/weekends) and travel to various worksites.
4. Technical Proficiency: Comfortable with technology, including the use of blood pressure cuffs and standard office software (Word, Excel). Experience utilizing REDCap and Epic databases.
Preferred Skills and Abilities
1. Education: Associate or Bachelor's Degree in a related field preferred, or an equivalent combination of education and experience.
2. Experience Requirements: 3 years of experience with a High School Degree, 2 years with an Associate Degree, or 1 year with a Bachelor's Degree.
3. Bilingual Proficiency: in both English and Spanish.
4. Preferred Specialized Experience: Three (3) or more years of experience working in community-engaged programs and research projects; demonstrated interest in addressing health inequities from a social determinants of health perspective.
Principal Responsibilities
1. Researches and collects data through site or home visit intakes; library research; structured interviews; or through other means for research projects. 2. Interprets, synthesizes, and analyzes data using scientific or statistical techniques. Modifies and plans research experiments, procedures, tests, or survey instruments. Assists in research design. 3. Writes and edits material for publication and presentation. Reports on status of research activities. Oversees and instructs research and support staff on technical procedures, equipment operation, and laboratory maintenance. 4. Schedules and coordinates research activities. Identifies and recruits study participants. Orders and maintains inventory of supplies. 5. May assist in preparing financial information and monitoring budgets. 6. Performs additional functions incidental to research activities. Health Requirements
This role is a healthcare worker position. Healthcare workers (HCW) are defined as university employees working a healthcare setting who have the potential for direct or indirect exposure to patients, human research subjects or infectious materials including body substance, contaminated medical supplies, devices and equipment, surfaces, or air. HCW have specific health requirements that must be met prior to starting work, including MMR vaccine or immunity, varicella (chickenpox) vaccine or immunity, TB screening, COVID vaccine according to University policy, hepatitis B vaccine or immunity, and annual flu vaccination.
